Full-Time
Main Areas of Responsibility: 

Provide person-centered, comprehensive case management/community support services to individuals with mental health and/or substance use diagnoses.

Additional Areas of Responsibility: 

Monitoring and coordinating treatment, linkage to community services, crisis intervention, advocacy, comprehensive care planning and skill-building activities, as well as appropriate and timely documentation.

Other Information/Requirements: 

Requires a minimum of high school diploma or GED and 3 years' relevant experience. Preferred: bachelor's degree in human services field (social work, psychology, counseling) and 1 year relevant experience or master's degree in a human services-related field. Other requirements: motor vehicle records check, drug screening and criminal background check.
