Main Areas of Responsibility:
Independently provide ongoing and direct community support services to individuals with severely disabling mental illness as part of an inter-disciplinary treatment team. Spend considerable time (at least 60%) in the community to engage/coordinate support services (family, physicians, psychiatrists) for each client as needed.
Additional Areas of Responsibility:
Perform a variety of casework duties including: analyzing/assessing clients’ psychosocial and multi-systemic needs; level of family functioning; formulation of individual clinical service/treatment plans for each client; implementation of services to assigned clients; provide advocacy for clients as needed.
