Community Support Worker

Job Description
Main Areas of Responsibility: 

Immediate opening for Community Support Worker to provide CCSS services to adults who have or are experiencing homelessness for our Comprehensive Recovery Treatment program. Responsible for providing CCSS services.

Additional Areas of Responsibility: 

Assist and advocate for clients with short term needs/supports and long-range planning in the following areas: housing, psychiatric, substance abuse, medical, community supports, legal, dental, ADL-s, interpersonal

Other Information/Requirements: 

Minimum BA in Psychiatric, Social Work or related field; 1 yr exp with Adult SDMI, Adult substance abuse &/or Adult Homeless populations; Conduct groups, reliable transportation for home visits; Computer literate; Excellent oral and written communication skills; Good organizational skills; Bilingual helpful
