Community Support Worker

Job Description
Main Areas of Responsibility: 

Therapeutic Living Services, Inc. is seeking a Community Support Worker to provide clinical case support to severely disabled mentally ill & homeless adults & their families. Office & home visitations required. As a member of the treatment team, perform a variety of casework duties including assessment of client's psychosocial needs; level of functioning; formulation of clinical service/treatment plans; implementation of services to assigned clients; & advocacy as needed.

Additional Areas of Responsibility: 

Build & maintain positive working relationships with other community agencies to facilitate treatment. Paperwork includes assessments, daily progress notes, clinical service/treatment plans, etc. Attend in-service & staff meetings. Maintain ongoing communication regarding treatment process with supervisor.

Other Information/Requirements: 

Candidate must have BA/BS in social service field and 1 year experience. Must be self-directed & possess excellent oral & written communication skills.
Must pass background investigation, Drug & Alcohol Screening, & TB Test.
Hours are Monday-Friday 9:00-4:00.
