Community Support Worker

Full-Time
Main Areas of Responsibility: 

Need two Community Support Workers to provide CCSS to homeless and near homeless SMI population; Initiate and maintain a therapeutic alliance with CRT Client; and help Clients achieve treatment and life goals.

Additional Areas of Responsibility: 

Complete initial program paperwork within 30 days of entry and updated every 90 days; Client assessments; Review Participation Contracts; Complete strength based quarterly Treatment Plans; Link Clients to psychiatric, substance abuse, medical, dental or other services needed; Teach Clients to advocate for themselves and to obtain community services.

Other Information/Requirements: 

Must have Bachelors degree in human service related field.
Must have at least one (1) year experience with adult homeless, SMI or SUD population.
Must have reliable vehicle to perform home visits
