Main Areas of Responsibility: 

At Communities In Schools, we are "all-in" for kids. It is our passion - it is our mission. We provide resources and create communities of caring adults who work hand-in-hand with educators to make sure young people have the tools they need to stay in school and achieve in life. We are seeking a Community Resource Navigator who will work closely with school-based Site Coordinators and community partners to connect students and families with community resources.

Additional Areas of Responsibility: 

Serve as the point of contact for Unite Us, a person-centered care coordination platform and a hands-on community engagement database.
Serve as main point of contact for CONNECT (the network of navigators at clinics, hospitals, schools, community service organizations and city and county programs that link people to services and resources).
Research and connect with community resources.
Support Site Coordinators with the Unite Us platform.

Other Information/Requirements: 

Community outreach experience
Preferred bachelor’s or higher degree in social work or related field
Case management experience
Required bilingual English/Spanish

Salary Range: 
$50k entry level to $70k for incumbent with at least 5 yrs experience + generous benefits package
