Community Health Worker

Job Description
Type of Job: 
Full-Time
Main Areas of Responsibility: 

Provide patient education, process referrals both internal and external and ensures follow-up. Will work with outside community based programs as needed. Will document activities into EMR as required.

Other Information/Requirements: 

High School or equivalent and 2 years experience working in an integrated social services/primary care environment providing patient education and referrals. Certified as a Community Health Worker or must start certification within first year of employment. Must be proficient in computers and data entry and have a valid NM driver's license with clean driving record. Bilingual (English/Spanish) preferred.
