College Readiness Project Facilitator

Full-Time
Main Areas of Responsibility: 

●Facilitate, prepare and follow up for college readiness and family engagement workshops
● Success Planning (preparation and meetings with students and families)
● Recruit, engage and re-engage families in college-bound success planning
● Manage family data for the purposes of case management and program evaluation
● Commit to SCC’s professional values and personal accountability by consistently following a work plan

Additional Areas of Responsibility: 

● Link families to existing academic, social, and health-related resources
●Build upon and formalize Southwest Creations’ referral network of schools, community organizations and service providers

Other Information/Requirements: 

●Fluency in English and Spanish (both written and spoken)
● Expedience working with families
● Ability to work towards project goals individually and with team support
● Strong facilitation and community outreach skills
● A Bachelor’s degree
