Main Areas of Responsibility:
Responsible for providing comprehensive assessment to determine eligibility of programs; develops client service plans; monitors and documents client progress, prepares reports, files, and advocates for clients as needed. Coordinates case management services with community agencies and other internal programs; monitors and documents clients' progress and prepares case notes and official reports; outreach to English and Spanish speakers.
Additional Areas of Responsibility:
This position prefers a BA/BS in Social, Behavioral, or Health Sciences and/or relevant experience in related field; one year minimum of experience working with under-served populations, intermediate computer skills in MS Office and Windows; and knowledge of Albuquerque social service systems. Bilingual in English/Spanish and multicultural experience is preferred.
