Case Manager/Service Care Coordinator - PT

Job Description
Main Areas of Responsibility: 

Provide Case Management and Service Care to residents in supportive housing situation. This position is part-time (20 hours per week).

Work in partnership with existing CM/Svc. Care Coord. to ensure that residents are provided with support and options to increase their chances for success in housing.

Additional Areas of Responsibility: 

Some emergency case management as needed. Work with existing CM/Svc. Care Cood. in preparing paperwork, reports as needed.

Other Information/Requirements: 

Minimum: Bachelor's degree in human service related field.
At least 1 year experience with adult SDMI or homeless population.
