Main Areas of Responsibility: 

We are looking for Housing Case Managers with passion for providing services within the homeless communities. Work directly with individuals and families who are experiencing homelessness. Providing wrap-a-round services, meeting clients where they are and serving our most vulnerable populations. Assist with completing applications for assistance to attain or retain stable housing to ensure our clients become self-sufficient in ending their homelessness.

Additional Areas of Responsibility: 

Provide comprehensive assessment to determine eligibility of programs w/individuals who are experiencing homelessness; develops client service plans; monitors/documents client progress, prepares reports, files, and advocates for clients as needed. Coordinates case management services w/community agencies and other internal programs; monitors and documents clients' progress and prepares case notes and official reports; outreach to English and Spanish speakers.

Other Information/Requirements: 

BA/BS in Social, Behavioral, or Health Sciences and/or relevant experience in related field; one year minimum of experience working with under-served populations, intermediate computer skills in MS Office and Windows; and knowledge of social service systems. Bilingual in English/Spanish and multicultural experience is preferred.
