Case Managers

Job Description
Main Areas of Responsibility: 

Provide case management to those experiencing homelessness and those recently housed. Provide resources and referrals, complete assessments, develop treatment plans, conduct home visits, prepare case notes and transport clients.

Additional Areas of Responsibility: 

Our socials services program is expanding their case management team.

Other Information/Requirements: 

BA in a social service field or equivalent experience and 2 years working with underserved populations as Case Manager. Working knowledge of EHR and experience with CMI & DSM-IV. Must have valid NM driver's license and clean driving record.
