Case Managers

Job Description
Main Areas of Responsibility: 

Our social services program is expanding their case management and is currently seeking Case Mangers to provide case management to those experiencing homelessness and those recently housed. Duties include providing resources and referrals, completing assessments, developing treatment plans, conducting home visits, preparing case notes and transporting clients.

Other Information/Requirements: 

BA in a social services field or equivalent experience and 2 years working with underserved populations as a case manger. Should have working knowledge of EMR and experience working with CMI and DSM-IV. Must have valid NM driver's license and clean driving record. Bilingual (English/Spanish) preferred.
