Main Areas of Responsibility: 

Position Summary: Provides case management services to persons served through service coordination, advocacy, evaluation, assessment and crisis management in an assigned program of responsibility.

Other Information/Requirements: 

Minimum Qualifications:Individuals must be able to pass a criminal background check and drug test. Individuals must have a valid New Mexico driver’s license, liability insurance, insurability under the agency insurance carrier, dependable vehicle and a clean driving record. •Bachelor’s Degree in social services or related human services field, Master’s degree preferred and/or nursing degree (BSN or higher).•One to three years’ work experience
