Main Areas of Responsibility:
As a Service Coordinator (Case Manager), you would provide case management for in-crisis, runaway and homeless youth who reside at Transitional Living Program (TLP); provide oversight of TLP systems and services including life skills education, file maintenance and program facility, equipment and vehicles.
Additional Areas of Responsibility:
Provide case management services from screening and intake to discharge including: completing client assessments; developing self-sufficiency plans, implementing, coordinating and reviewing; and providing discharge and after-care planning and implementation for an assigned case load. Facilitate leadership opportunities and activities to facilitate clients' achievement of their personal goals.
