Case Manager/ Service Coordinator

Job Description
Main Areas of Responsibility: 

Full time position. Assist families with infants/toddlers who have developmental delays/disabilities to access and coordinate community based services for their children.Conduct enrollments, write service plans, monitor service delivery and support transition to other services. Coordinate services with therapists, physicians, health care providers and school personnel.

Other Information/Requirements: 

Must be able to drive to provide in-home services in Bernalillo, Rio Rancho, Paradise Hills and Ventana Ranch areas. Seeking a friendly, very organized, detail oriented individual. Must have BA/BS degree in education, social work or related field. Prefer Spanish speaker. Competitive salary and fringe benefits.
