Main Areas of Responsibility:
Housing Case Managers w/a passion of providing services within the homeless communities. Work directly with individuals who are experiencing homelessness; provide support in completing applications for assistance to attain or retain stable housing. Meet w/housing participants on a regular consistent basis to provide ongoing wrap-a-round services. Familiarity in Housing First, Harm Reduction, Trauma Informed approaches & proficient in MS Office application.
Additional Areas of Responsibility:
Responsible for providing comprehensive assessment to determine eligibility of programs with individuals who are experiencing homelessness; develops client service plans; monitors and documents client progress, prepares reports, files, and advocates for clients as needed. Coordinates case management services with community agencies and other internal programs; monitors and documents clients' progress and prepares case notes and official reports; outreach to English and Spanish speakers.
Salary Range:
Pay range starts at $18.18 per hour, DOE
