Case Manager

Job Description
Main Areas of Responsibility: 

Provide case management services, including providing resources and referrals, completing assessments, developing treatment plans, conducting home visits, writing case notes and transporting clients.

Other Information/Requirements: 

BA in Social Services field or equivalent experience. 2 years experience working with underserved populations. Working knowledge of EHR, and CMI and DSM-IV. Must have NM driver's license and clean driving record. Bilingual (English/Spanish) strongly preferred.
