Main Areas of Responsibility: 

This position is responsible for providing comprehensive assessment to determine eligibility of programs; develops client service plans; monitors and documents client progress, prepares reports, files, and advocates for clients as needed; and coordinates case management services with community agencies.

Additional Areas of Responsibility: 

This position is responsible for other internal programs; monitors and documents clients' progress and prepares case notes and official reports; and outreach to English and Spanish speakers. This position prefers a BA/BS in Social, Behavioral, or Health Sciences and/or relevant experience in related field; one year minimum of experience working with under-served populations.

Other Information/Requirements: 

Intermediate computer skills in MS Office and Windows; and knowledge of Albuquerque social service systems are preferred. Bilingual in English/Spanish is required; and multicultural experience is preferred. Pay range starts at $17.30 per hour/$35,984 annually, DOE.
