Main Areas of Responsibility:
Responsible for providing comprehensive assessment to determine eligibility of programs; Develops client service plans; monitors and documents client progress, prepares reports, files, and advocates for clients as needed. Coordinates case management services with community agencies and other internal programs.
Additional Areas of Responsibility:
Monitors and documents clients' progress and prepares case notes and official reports. This position prefers a BA/BS in Social, Behavioral, or Health Sciences and/or relevant experience in related field; one year minimum of experience working with under-served populations required,
