Job Description
Main Areas of Responsibility: 

Provides case management services to persons served through service coordination, advocacy, evaluation, assessment and crisis management in an assigned program of responsibility.

Other Information/Requirements: 

Individuals must have a valid NM driver’s license, liability insurance, insurability under the agency
Bachelor’s Degree in social services/related human services field, Master’s degree preferred and/or nursing degree.
One to three years’ work experience in case management or social work is required
CRC, CCM or MSW preferred
Bilingual preferred
