Main Areas of Responsibility:
Provides comprehensive assessment to determine eligibility of programs; develops client service plans; monitors and documents client progress; prepares reports, files and advocates for clients as needed.
Additional Areas of Responsibility:
Coordinates case management services with community agencies and other internal programs; monitors and documents clients' progress and prepares case notes and official reports. BA/BS in Social, Behavioral or Health Sciences required. Bilingual in English/Spanish preferred. Intermediate computer skills in MS Office and Windows required.
