Case Manager

Job Description
Type of Job: 
Full-Time
Main Areas of Responsibility: 

Provide case management, resources and referrals to individuals experiencing homelessness. Job responsibilities include completing assessments, developing treatment plans, conducting home visits, writing case notes, and transporting clients.

Other Information/Requirements: 

BA in social services or equivalent experience and 2 years working with underserved populaltions. Working knowledge of EHR. Experience with CMI and DSM-IV. Certification in CCSS preferred. Must have valid NM driver's license and good driving record. Bilingual (English/Spanish) preferred.
