Main Areas of Responsibility:
The primary duty of the Case Manager is exercising good judgment in the specialized area of counseling and crisis intervention and providing support services to parents engaged in the Family Wellness program. Help families find resources: problem-solving, housing, income support, disability benefits, education, life skills management, etc. CM also provides parent education and coaching, based on the Nurturing Parenting model utilized in Family Wellness individual and group therapy.
Additional Areas of Responsibility:
1. Apply training and specialized knowledge.
2. Provide services in the community and home settings.
3. Document/progress notes in a timely manner.
4. Contribute to program, evaluation and continuous Improvement.
5. Coordinate with other agencies.
6. Appear in court and other venues to represent All Faiths.
7. Identify and provide crisis intervention and stability services.
8. Participate in training, supervision, and consultation.
