Case Manager

Job Description
Type of Job: 
Full-Time
Main Areas of Responsibility: 

Conducts intake assessment interview to determine eligibility for program services.
• develops client service plans
• monitors and documents client progress
• prepares reports
• files
• advocates for clients as needed

Additional Areas of Responsibility: 

Coordinates case management services with community agencies and other internal programs.
Monitors and documents clients' progress and prepares case notes and official reports.

Other Information/Requirements: 

BA/BS in Social, Behavioral, or Health Sciences or an Associate’s degree preferred. Basic computer skills in MS Office and Windows required. Knowledge of Albuquerque social service systems, multicultural experience preferred. One year minimum of experience in related field required.
