Case Manager

Job Description
Main Areas of Responsibility: 

To provide case management, resources and referrals to people experiencing homelessness. Complete assessments, develop treatment plans, conduct home visits, write/update case notes and transport clients.

Other Information/Requirements: 

BA in a social services field or equivalent experience and 2 years experience working with under-served populations. Working knowledge of electronic health records and experience with CMI and DSM-IV. Must have a valid NM driver's license and clean driving record. Bilingual (English/Spanish) strongly preferred.
