Job Description
Main Areas of Responsibility: 

Coordinates with medical, mental health, substance abuse, and other staff to improve health and quality of life outcomes for high-risk HIV-infected patients by identifying and overcoming barriers to medical care and treatment adherence.

Other Information/Requirements: 

Bachelor's and/or Master’s Degree in related field; or four to six years related experience and/or training; or two years of experience in social services, finance or equivalent preferred; or equivalent combination of education and experience.
