Case Manager

Job Description
Main Areas of Responsibility: 

Provides case management services using assessment, linkage and coordination, follow-up and advocacy.Provide case management services to assigned caseload. Approves and submit monthly billing. Complies with state/contract guidelines and regulations. Documents in a timely manner all activities through narrative statistical and financial reports. Ensures HIPPA compliance. Bilingual is a plus.

Additional Areas of Responsibility: 

Interact and maintain good working relationships with individuals of varying social and cultural backgrounds and people with disabilities. Must possess a New Mexico drivers license with liability insurance; must have a dependable vehicle and clean driving record. Have the ability to pass a criminal background check, FBI fingerprinting and drug/alcohol test.

Other Information/Requirements: 

: Bachelor Degree in counseling or related human services field, Master’s Degree preferred and/or nursing degree (BSN or higher). CRC or CCM preferred. Licensure as appropriate. 1 to 3 years experience in rehabilitation counseling or case management. Experience completing Medicaid Waiver, Mi Via, Colts, PCO, Medically Fragile, DD Waiver, etc. assessments.
