Main Areas of Responsibility:
The Case Manager - Shelter provides direct services to shelter residents These services include, but not limited to: legal, community and social advocacy, education and support, crisis intervention and transportation. Shelter Case Managers are assigned residents as they come in to shelter.
Additional Areas of Responsibility:
-Develops and implements individualized service assessments and plans with residents.
-Provides information and support to residents and program participants regarding accessing community and agency resources required to achieve individualized goals.
-Works with Therapists, Resident Advocates and others in the facilitation of individual resident service plans.
-Provide reliable information in spoken and written communications.
-Conducts education and training classes as needed..
