Main Areas of Responsibility: 

ACES Navigator will maintain a caseload of potential participants, ensure connection to support services, and secure eligibility documentation. Once CES participants enter housing, Navigator will conduct follow-up assessments to maintaining housing and connect to other community supports. The Navigator will provide outreach to shelters and street outreach. Position is based out of the NMCEH Albuquerque office but requires travel to member agencies and occasional travel to NMCEH Santa Fe office.

Additional Areas of Responsibility: 

Conducts common assessment tool over the phone and in-person. Triages households experiencing homelessness or housing crisis; engage, assess capacity and motivation, and navigate people seeking housing and services; maintain contact and gather durable reconnection information; manages and tracks new referrals and ensures timely placements; gather, complete, and submit eligibility documentation and applications for housing; and complete all relevant data entry and maintenance in the HMIS.

Other Information/Requirements: 

Occasionally utilize personal vehicle to meet with people seeking services, community members, and agencies; support gathering resource information in the community; connects households to supportive services as needed; triage and prioritize phone calls, emails, and texts; coordinate with program managers and direct care staff from various agencies that utilize the Albuquerque Coordinated Entry System; and maintains confidentiality of all privileged information.
