Mission Statement:

A future that honors culture & tradition where Indigenous communities are healthy & strong. Healthy Native Communities Partnership, Inc., (HNCP) works with communities to realize their own vision of wellness. We support community wellness,
leadership development and networking from the wisdom and strength of local communities.

Primary Programs:

NATIVE WELLNESS RESOURCE NETWORK - The Network supports Tribes and Native Communities to share best and promising practices for wellbeing. Members tap into a rich
network of Resources including trainings and proven tools in public health, leadership development, and participatory community wellness planning.
NATIVE HEALTH COMMUNICATIONS - We support Native community voices for wellness through capacity building with Digital Storytelling. Who is telling your story? As Native people, others have told our stories filtered through their own lenses. The power of authentic storytelling strengthens health and wellbeing, and preserves Native culture.

Other Programs:

Community Connections Through Hozho, a collaboration- Shiprock/UNM Family Medicine Residency Program
HNCP collaborates with the Shiprock Family Medicine Residency program to aid with connecting Family Medicine Residents and project staff to Navajo Chapter communities. Our aim is to strengthen the community enrichment experience that engages both the doctors and community members to address community needs and challenges based on community strengths.
Sih Hasin Street Medicine Program
The Sih Hasin Street Medicine Program is a mobile health care initiative launched in October 2021 with a goal to bring medical, mental health, and wellness care directly to unsheltered relatives and individuals experiencing homelessness in rural reservation communities. HNCP provides administration oversight and collaboration activities with the Sih Hasin Street Medicine Program.
