First Choice Community Healthcare is dedicated to improving the health, life skills and well-being of all members of the communities we serve.
Primary Programs:
We provide primary medical care, dental care and WIC services and have integrated behavioral health care in the primary care setting for our established patients. We ask for payment at the time of service and help people to apply for assistance to help cover their costs. Extended hours are available to established patients. Appointments are necessary.
