Main Areas of Responsibility:
Barrett Foundation Housing Case Managers partner with individuals and families experiencing housing instability to identify, obtain, and maintain safe, stable, and habitable housing. Through a collaborative, strengths-based approach, Case Managers work alongside each household to develop and update a Housing Plan at least quarterly, identifying goals, available resources, and steps toward long-term housing stability.
Additional Areas of Responsibility:
Meet with program participants via home visits, office visits, field visits, telephone, email, or texting
Conduct program participant screenings, intakes and support housing placement activities.
Ensure that participants are provided with relevant community resources and information about healthcare and other supportive services
