Main Areas of Responsibility: 

HopeWorks is seeking to fill various CSW and CSW related positions in different departments within the organization. The basics of the CSW position is to assist clients in securing treatment programs, and other services provided by HopeWorks as well as assisting them in navigating life such as securing IDs, Medicaid and other programs.

Additional Areas of Responsibility: 

Completing discharge plans and yearly crisis plans for clients as well as preparing periodic assessments of programs and facilitate group meetings as appropriate. Preparing notes and enter billing in a timely manner and in accordance with agency and program guidelines.

Other Information/Requirements: 

Bachelor’s degree, preferably in human services related field.
At least one (1) year experience in case management or direct care working with SDMI population or other relevant experience.
